Below are the forms that will be required for registration at Barkalounge. You may fax them to us at (317) 788-8552 or print them and bring them by in person. Prior to acceptance, all dogs will go through a one day evaluation and temperament testing.  Please call in advance to schedule your dog’s first day!

Verification of Health Records
All dogs must be in good health and if over 6 months of age, must be spayed or neutered. We require a copy of veterinary records showing your dog is current on all vaccinations for:

  • Distemper/Parvo (DHLPP)
  • Bordatella (required every 6 months)
  • Rabies
  • Protected from fleas, ticks, and heartworm
